Anyone in Florida know if you have to have a signature in Box 31 on a 1500 claim form?

I'm in California, but I have a question:  does the Workers Comp. carrier in question accept electronic billing?  If they do, then the answer to your question would be "no".  Even when we send paper claims to Workers Comp. in California, we only print the name of the physician.  There is never an actual signature there.

I'm in MN but I very much doubt it, we deal with worker's comp insurance based out of pretty much every state in the country and they almost all require paper submissions. No signatures (either hand-written or otherwise) go on the forms. They've never asked for them. If they also request the progress notes for each claim, a signature is expected but e-signing is fine there too.

I agree with both responses.
