: New Psychotherapy codes for 2013

I just read an article that CPT code 90862 (which is medication management for psychiatry) is being eliminated and that "Prescriber-level providers must learn Evaluation and Management Guidelines in replacement of the Medication Management code."

I was in another forum (I'm a member of many) and posed the same question.  Many people did not know anything about the changes.  There was, however, one person who did.  She said that 90862 is being eliminated and that the other mental health codes are changing...she provided several codes with descriptions.

I then contacted a provider rep for BCBS of MI, she too had no idea of the changes.  I searched the BCBS and Medicare websites and was not able to confirm any of the codes.   :(

Has anyone heard anything about these changes?

I appreciate any help you might have.

Yes there are changes, I got something from a client that I think came from the APA, so I will type it out here, it's a chart.

Initial Psychiatric diagnostic eval (formerly 90801) or new patient E/M Code
       *90791  Psychiatric diagnostic eval (no medical Services)
       *90792  Psychiatric diagnostic eval (with medical serivces)  New patient E/M codes may be used in lieu of 90792

Psychotherapy (formerly 90804-90808, 90816-90821) For use in all settings; time is with patient and/or family
    *90832  Psychotherapy 30 min
    *90834  Psychotherapy 45 min
    *90837 Psychotherapy 60 min

Evaluation and Management E/M and Psychotherapy (Formerly 90805-90809), 90817-90822)
    *Appropriate E/M code (not selected on basis of time) and +90833, 30 min psychotherapy add on code
    *Appropriate E/M code (not selected on basis of time), and +90836, 45 min psychotherapy add on code
    *Appropriate E/M code (not selected on basis of time), and +90838 60 min psychotherapy add-on code
 
Medication Management (formerly 90862 or E/M Code)
    *Appropriate E/M code (99xxx series)

Interactive Psychotherapy (formerly 90802, 90810-90815, 90823-90829, 90857)
For use with the psychiatric eval codes, the psychotherapy and psychotherapy add on codes, and the group (non family) psychotherapy code
    *90785  Interactive Psychotherapy

Crisis Psychotherapy (NEW)
    *90839  Psychotherapy for crisis, first 60 min (Appropriate E/M Code may be used in lieu of 90839)
    *90840  Psychotherapy for crisis, each additional 30 min


I don't know about anyone else but I see some problems with these. LOL   :-* ::)

They think they are keeping us on our toes I suppose.. guess they don't realize we have to run on those toes as well!!

We bill quite a bit of mental health so I'm waiting to see how it plays out too!

When exactly are these codes to take effect?  is this 1/1/13 or the October 2014?

1/1/2013.  :)  They will still be part of the normal 90 day grace period I'm assuming.  ...then again.. never assume in this industry.

Hello there,
I just attended a webinar regarding these codes. We were all told there is no grace period. We have to be on are toes starting 1-1-13....Yikes :-X

When dealing with workers compensation, are they going to update their codes for this?  I am just wondering since most of the w/c companies still use the old testing codes of 96100 instead of 96101, etc.

You will have to get in touch with each carrier. I'm JUST guessing here but since there is no grace period I'm thinking they will all be updated but that's probably in a perfect world.

When using the 90785 add on code with 90637, is any money posted to that code or does all of the money go with the main code?  Does it halep to put the amount of time spent face to face with the client to help justify the additional cost.  Do you add units to the main code or to the add-on code?
