Billing for Surgical Assists

I currently perform the billing for providers who are Surgical Assist (SA) in Texas, who set up to do their own billing in 2009. I had just finished a 5 month course in billing and coding and SA's was not addressed in the course. I was hired from the beginning to do the billing for them. I have basically had to learn on my own and still learning. Government and subsidies insurance does not recognize SA's. I know that SA's are out of network may or may not get paid. Appeal or not to appeal is an issue. Often, I see there is a deductible and patient responsibility.

I am wondering if there are any members in this forum who are/have billed for SA's and have some general billing knowledge for this type of provider.
