Sorry but I'm not familiar with one. Won't the one you posted work for all CPT codes?

The CPT codes available in this calculator include those codes most frequently used for the care of spine patients to include evaluation and management, spine, cranial, injection procedures, and diagnostic procedures. The Medicare allowance will calculate based on the 2012 published RVUs and a conversion factor of $34.0376.
I don't know of another besides the AMA has for members only. I do know if you do a search of spreadsheets you will find one you can download maybe? I am pretty sure that's all it is (spread sheet)

Yes, you are right.
Another question, if the fee schedule does not include a CPT code, what does this mean? An association detailed all the code changes for 2013, however, when I cross referenced the codes on the 2013 schedule for the area, some of the codes were not listed.
