Starting Small Group Practice

Hi all,

I have a client (LMHC) who has a group NPI and TIN.  Right now, he is solo and credentialed with many insurance panels. He wants to hire another LMHC to his group to also be credentialed. I am making a list of what to do prior to contacting insurance companies. Am I missing anything?

1) CAQH for new provider
2) license for new provider
3) W9  - group name and group TIN?
4) malpractice- new provider carries own malpractice? or is there a such thing as group malpractice?

No such thing as group malpractice though some groups have group coverage as a "SUPPLEMENTAL" type of coverage but it's more like E/O (errors and omissions) Each provider must carry their own policies.

what goes on line # 1 of the W9?  indiv provider's name or group name?

If you are doing the W9 for the business (which is what it sounds like) then the group name goes on line 1.

as far as credentialing a brand new (never credentialed provider ) into a group, when the applications require a W9- does the provider's name appear on the W9 at all?  Or just the group name?

also- I assume that the group owner signs the W9?

The W9 is the information regarding the EIN/Tax ID.  Since that is the group, the individual's name is not listed on there.  The W9 must be signed by an authorized representative of the group, which in many cases is the owner.
