PT REFUNDS

I have had this come up a few times lately and wanted to see how to handle this.  A few patients that had refunds coming to them (generally a small refund of $5 or $10), have stated not to bother sending it to them.  Patients have said, "give it to a charity of your choice."    Is this possible?  If so, would I just write a letter to the patient (and keep a copy) detailing that they have a refund due to them but at their request we are donating to ABC Charity?

I have had that come up too. We actually tell the patient they are free to donate the fee and cut them a check. It's too much of a hassle. We just nicely tell them that's really nice of them but for accounting purposes we need to issue the refund and they are free to do what they want with it. You don't want the extra headaches and writing it off doesn't seem like an honest thing either in that case.  I have run across this with our mental health patients and most of them will come in weekly so many times we just credit the copay for the following visit. If it's obvious in the chart they have hit 100% coinsurance or something like that we just cut them a check next time they come in or mail it if they don't have another visit scheduled.

This actually reminds me of a program one of my clients ran for abusive men and sex offenders, their insurance would pick up charges but the courts would actually require payment from them and many of them wanted us to just credit their copay's but we couldn't do that, it was up to them to make the appropriate payments if they had a credit.
