Billing services

Hi, I am thinking about venturing into a private medical billing service.  How would I bill?  I realize that the provider has to be entered on the CMS 1500 but then do I bill with my billing service in box 33 or do I just bill under the physician's name with the "billing" address as my companies??  This may sound dumb but I want clean claims from the get go and have always worked for the hospital, physician, center to do their billing.

The address in box 33 should be the correspondence/pay to address for the provider.  If the checks are going to your billing service, then your address would go there, but if the provider gets the checks directly then their address goes there.  The provider or the group name always goes in box 33.  They don't really care who the billing service is, they want the name of the provider/group.
