: Type of Bill Box 4 on UB04 Form

Question
I have a question about completing a UB04. As you know, each insurance company has its requirements about what information is needed to process the claim. For the most part, we are able to correctly complete them, except for one box, #4 Type of Bill! Could you explain this box? ; We are a small facility, and bill for a small group of surgeons who perform only a few vascular surgeries.

According to the "UB92 Editor"  Box #4 Type of Bill provides specific information about the bill. The first digit identifies the type of facility. The second digit classifies the type of care being billed and the third digit indicates the sequence of the bill for a specific episode of care.

Once you determine the correct numbers, they aren't likely to change. The number 8 indicates special facility or ACS. A 3 for the second digit would mean Ambulatory Surgery Center. For the third digit a 1 indicates admit though discharge, a 2 indicates the first claim of a series, a 3 indicates a continuing claim in a series, and a 4 indicates last claim in a series.

To us, based on the information you provided in your question, we would use 831. We hope this information is helpful.

Our facility purchased your book UB04 Forms, How to Complete a UB04 Form Completely and Correctly and its been extremely helpful.  Recently we learned of another code 0490 (for box 42) but it is not listed in your book.  Would you have any information about this code?  As always, thanks for your help.  Nancy O./ MAJ Surgery, PC

0490 is:  Ambulatory Surgical Care: General Classification

They are always adding/changing!  Hard to keep up.  Glad to hear the book is helpful.
