How do I bill 3 pair of diabetic shoe inserts?

There are people here who are more knowledgeable about DME than I am. But, I question whether Medicare would allow three pairs of these. And the diagnosis must support the necessity of the inserts..The claim must go to the carrier that handles DME and the supplier needs to be a Medicare provider. These claims do not go to the regular MAC.
