? A/R cleanup only

How should I charge a physician who just want to outsource there A/R that's over 60 days should it be a flat fee or percentage

It's your business right? Charge whatever you want taking into consideration state laws.  I will say this.. taking on A/R recovery is a lot of work.. if you go by a percentage, more likely you will work pretty hard for very little money.. JMHO     For A/R recovery I usually take a good look at it and I might go flat fee if it's a long term project or hourly if it's not.  NEVER a percentage.

You could do a down payment and then a percentage of what is collected from what you bill for.  The down payment needs to be a good down payment too, not $100 or something else ridiculous as A/R Recovery is a lot of work/ clean up.

Ditto!  Depending on what insurance companies are involved and WHY the claims weren't paid to begin with it could be super easy or extremely difficult to get anything at all.
