PAIN MGMT - Radiofrequency (RF) SI Joint

I am getting conflicting information on how to bill out the SI RF.  We have been using code 64622, however an article found states we should be using 64640...anyone have any thoughts on this? Thanks so much for the input. Mary Kaye

I don't have any personal experience in billing this, but I found the following information:

64622(Destruction by neurolytic agent, paravertebral facet joint nerve;
lumbar or sacral, single level) x 1 unit

64640(Destruction by neurolytic agent; other peripheral nerve
or branch) x 3 units

I understand the descriptive of the codes, it is anatomy I need assitance with to know which code is correct.  I am reading the paravertebral facet joint nerves don't innervate the SI joint.  Anyone familiar?
