Incident to Billing for Pain Practice

My questions is, we are going to bill incident to for the PA that works in the office.  the physician is always there when the PA is seeing follow ups.  All other rules are applied, doctor sees patient first, establish care plan, etc...my question is, as I can not get a straighter answer from Medicare-Noridian....if we bill the office visit incident to under the physician...the office runs labs each time.  Do I bill the office visit under the doctor and the labs, etc under the PA's name.  when I look at medicares guidelines it says labs, ekg that incident to rules don't apply but I am worried about sending OV under the doctor and the labs, etc under the PA's info...
