need confirmation about date of illness

Can someone please confirm this date of illness dates

Superbill/fee slip Dos 4/2/14   
Treatment therapy services dx given 722.4,724.4,726.10   
date of illness 4/2/14

Superbill/fee slip dos 5/7/14 
Treatment  office visit only cx given 440.9 
date of illness 5/7/14


Superbill/fee slip dos 5/20/14
Treatment therapy services dx given 723.1, 723.3, 722.4, 728.85
Question should the date of illness go back to 4/2/14 (since some of the same dx were given)
Or
Should the date of illness be 5/20/14

Even if the dxs are the same, doesn't necessarily mean it goes back to the 4/2.  There unfortunately isn't enough info here for us to answer.  For example, "pt comes in on 4/2 complaining of shoulder pain/back pain.  Pt states they were raking leaves and felt pain in their back and shoulder."  and then for 5/20 "pt came in for neck and back pain.  Pt states that they were riding on a gokart and they got rearended by another gokart".  See the dilemma?  You can't assume that the care on 5/20 is or is not related to the care on 4/2 without seeing the patient's chart or being told by they provider.

I hope that is helpful but I'm afraid I just made it more confusing.
