Can anyone share how to determine how much to bill for the service fee for a substance abuse facility?  I have been in business for 7 years and all our clients are private practice.

Can you explain how sentence 2 is related to sentence 1 please.  Providing that detail will help generate more specific and useful answers.  (That is, you are in business and you have clients.  How are those two facts related to the substance abuse facility?)

I apologize for being unclear so please disregard the second statement.  I never billed for a facility so I am not sure how much to charge.

Can anyone share how to determine how much to bill for the service fee for a substance abuse facility?

Are you a biller wanting to know how much to charge a substance abuse facility for doing their billing?  Or are you trying to figure out how much to charge the folks who utilize the facility?  The phrase "service fee" is confusing things.

Is the facility in-network with health insurance carriers?  Is it cash-only, so you won't be billing insurance carriers?  Does the facility accept Medicare patients?

It is unclear which you mean but I'm going to answer as if you are a billing service looking to determine how much to charge:

We do facility billing for residential facilities.  In some cases we have a sliding flat fee schedule per month based on the number of beds that are filled.  This works well, especially if the facility is just starting out.  The amount of work is directly related to the number of patients they have. 

If they are out of network we sometimes set up a relationship where they actually refer the patient to us and we work directly with the patient (of course with the facilities help).  In this case we charge the patient a flat fee which includes the submission of the claim, and resubmissions necessary and assistance to them in determining what's happening with their claim.  Of course no guarantees that they will be reimbursed with out of network.

1.  Do you require payment from the patient before you submit their claim?

2.  If you bill the patient for your fee, do you have much difficulty collecting that fee (e.g., do most patients pay)?

Yes

We require they pay PRIOR to us submitting the UB04
