Provider going on her own

I met with a provider today who is considering opening her own office. She has been billing under another provider, but has her own NPI. I suggested she talk with her attorney and accountant regarding the best business structure for her. Question, what will she need to do about notifying insurance companies she bills? Will she need to recredential? Since she's already on record with insurance cos as the rendering provider, will notification of change of address & business name be sufficient?
BTW, as well as a general discussion, we also discussed EHR/EMR and I gave her a bunch of information on research I had done. She said she had met with a practice management consulting business in the area and halfway through our meeting, said I had already given her 10 times more information than they had. I think I made a good impression. If she goes on her own, I think she'll be a new client.

Some companies will be just a change of information and some will require new contracts.  Usually it is easier to change the info than to start from scratch.   You should contact each carrier that she wants to be in network with to see what they require.  Medicare will require the appropriate CMS form. 

It was wise to refer her to her attorney and accountant to pick the best business structure.
