What is the rule for refunding small balances, under $1.00?

You can do small credit adjustments, but you might want to check your state laws, I believe you don't have to send a check if it exceeds the cost of postage here in NY. If it's really old you could adjust it off, I don't think anyone will be knocking on the door for a $1.00 I would just put a note in the patients file JUST in case they ever do come back for their $1 :)
