If we have a signed hardship form and a documented hardship case, is it OK to write off the patient's deductible as well as their co-insurance amount?

Yes, but you want to follow the office financial policy to make sure the procedure for hardship cases is consistent and fair across the board. You should not have a hardship documented with separate rules/procedures. There should be ONE policy for the office from which to follow for financial hardship. Once documented and included in the patient's file you can indeed write off balances as stipulated in your office financial policy.
