Hi Michelle,

If the doctor sees a pt. for gen. medicine and physical therapy on the same day would I use modifier 25 or 51 on the E & M code ? That also brings up the use of 59 I was using mod 59 on physical therapy codes, specifically bilateraly treatment of legs, elbows, knees, shoulder, and feet, is that correct ? what is the best way to use these 3 modifiers ?

I attended a billing workshop this weekend that was specifically on physical therapy billing and was told to use 59 in the event of an Initial Evaluation for different body parts and to use 52 for treatment to let the payor know that they are still treating the different body parts but at a reduced rate.

You would use the 25 mod on the E&M code for the general medicine portion of the visit. 

Also, the 59 modifier is appropriate for the PT services.  I haven't used the 52 modifier before to indicate different body parts but it does indicate reduced services.  If that is what they told you at the workshop I would try it.
