Pre Authorization

Have any of you heard of or do any of you as a billing company call and start pre authorization on a client and "provide clinical information" when you have not seen the patient? You just go off of what the family might have told you to try to get authorization for RTC or Wilderness Programs?
Doesn't the patient have to be seen by the clinician in order to provide current clinical information to the insurance once the patient admits?

Usually any preauth being done before client is seen only requests reason appt is being scheduled.  We avoid the preauth process but have gotten involved a bit.  But in our experience the information requested is not usually very specific since patient hasn't been seen yet.
