Fissurectomy codes
« on: October 08, 2012, 05:24:05 PM »
1-I need to provide CPT codes for an anal fissurectomy with an anal advancement flap. Can I use 46200 even though a sphincterotomy was not performed? Is a 2nd procedure code required for the flap graft?
2- I’m searching for a CPT procedure code for an anal fissurotomy (without sphincterotomy)- sphincter sparing treatment of an anal fissure.  I’m thinking I need to use code 46999. Any thoughts?

I believe the 46200 states  "Fissurectomy, including sphincterotomy, when performed" which means it would still be appropriate if the sphincterotomy isn't performed.

I found the following information regarding whether the flap is coded separately:  http://www.codapedia.com/article_502_Coding-Excisions-and-Wound-Repairs.cfm

I do not find a cope for anal fissurotomy so it sounds like the unlisted code may be your only option.

Hope that helps!
