Medicare form 855I or 855B

Hi everyone, thanks in advance for help!

So I have a 35 year old non-profit expanding it's line of business as described in "need credentialing quote" topic.

I am thinking they need the 855B application that wouldn't require fees as opposed to the 855I for physicians and non physician practitioners which I think of as APRN etc.

According to the CMS Fee Matrix, the following is specified for the types of disciplines being used by the non-profit.

Outpatient Physical Therapy/Occupational Therapy/Speech Pathology
Services Provider that enroll via the CMS-855B

Question, how many forms do I fill out and which ones?
a. For the nonprofit itself
b. for each and every OT,PT,SP?

One form for all or individual forms for each?

You need the CMS 855B, 588EFT and the 460 for the group.

Then you need an 855I for each therapist to enroll them (if they are already Medicare providers you do not need this)

AND you need an 855R for each therapist to reassign their benefits to the group.
