Optometry

Anyone bill optometry? Most of my experience is psychology, however I have a Huge optometry client and want to make sure his billing is completely accurate. How often do your optometrists use the 99214 code? What codes are normally used?  I've been researching this a while but I'm still a bit clueless.  He also does vision therapy, those exams are much more extensive, but was wondering if 96116 could be billed with a 99205? I believe a modifier would have to be used.  Thank you!

We bill for several OD's.  We like Optometry billing.  The most common codes are 99202, 99204, 92012, 92014, 92225, 92250, 92083.....That's the most common.  There are things you need to know.  For example, Medicare requires that 92250 & 92083 have a referring physician.  The OD can refer to himself that the patient have the test but you have to list them as the referring physician.  For example, Dr Glance sees Betty for an exam and determines during the exam that she may have glaucoma so he wants to do a 92083.  He then performs the 92083 with himself as the referring dr.  Sometimes they will bill E&M codes, mostly when a patient has conjunctivitis or a corneal abrasion and they have to be seen every couple days just to be looked at.  Not really familiar with the vision therapy so I can't help you there.  It's not too hard!

I agree, it's pretty easy!

I was just mainly unsure of the E/M coding, because he always codes eye exams with medical diagnosis under 992x4, I wasn't sure if that was the norm for optometrists or not.  They are 60 min appointments and very thorough.  We do use the 92250 and 92082(83) codes also for the photos/vision field.  I was unaware of the referring provider with medicare, though.   

As I previously stated, I worked with psychologists and therapists before, so we never used E/M codes (mental health has got to be the easiest, imo!).  I ordered a pre-recordered webinar, but wondered if you guys had any Optometry guides in the works or know of where I could get some?  Or maybe you guys could do a phone training session?  I've purchased about 4 of your e-books and love them!
