81000 being denied

Medicare has denied procedure code 81000 as missing/incomplete/invalid referring
provider primary identifier. We are the patients primary care physician. We did the
urine dip stick as the patient was having symptoms of a urinary tract infection.

I have called Medicare and they were so rude! The rep told me that I need to call another
number which she provided, but when I called it was an automated service that gave me
the same information on the Medicare EOB.

Are you sure the 81000 is the correct cpt code?  Is it possible the 81002 is the correct code for the service you are doing?
