90837 & 90847

Hello,

I seem to have confusion with the two mental health codes 90837 and 90847. And I guess 90846. Since the new 90837 covers the patient AND/OR a family member, when would you use 90847 or 90846?

90837 in individual therapy Patient only
90846 is family/group WITHOUT patient
90847 is family/group WITH patient

90846/90847  ( family psychotherapy codes, ) the focus of the service  is on family dynamics or interactions – or a subset of the family such as parents or children
treatment is still intended for the benefit of the patient.

With the individual therapy codes, the focus is on the patient even though the CPT code descriptions for 2013 no longer include the word “individual” before “psychotherapy”. The codes can be used with the occasional involvement of family members.
