I'm interested in hearing from people who have personally filed health insurance claims (or know someone who did) for which the insurance company should have paid at least some money. Either at work or in your personal life.

I'm especially curious about Anthem or Cigna, but I'm interested in other insurers too.

Did they deny any part of the claims? If so, how many hours were spent appealing the decision and going to arbitration? Did the insured person have to drive to a distant place to participate in the arbitration hearing?

Please tell us all about your experience. It would be useful to know how much effort is involved in trying to actually use health insurance.

You really need to specify why you are asking.  Your questions are different from the normal questions we are asked.  Normally people don't mind helping/sharing but when we are unsure as to what is being asked and why, we are leary.
