Billing out for forms?

I was just wondering if the primary physician that has to fill out a certificate of medical necessity cms-484 oxygen form, if we can bill out for that. If we can does anyone know the cpt code.I know when he fills out forms for home health care we can bill out with G0180.

I'm assuming the Dr is not actually seeing the patient but just completing the forms. If so, there is a cpt code for report writing, 98012, but whether or not it is reimbursable will vary from plan to plan.
