What is a fair fee to verify benefits, the form has 12 questions? Does the insurance carrier allow you to, fax the vob form to them? How long does it take the insurance carries, to return the completed vob form?

"What is a fair fee to verify benefits, the form has 12 questions?"  -  This sounds like you are referring to a specific form for one carrier.  I would determine how much time is involved (total, filling out form, faxing and tracking) and base your fee on that time.

I would say what you charge first should be determined by how you have your contract setup. If you've not yet signed the client then you can determine first how you are going to charge.  Per claim, per service, hourly, monthly, percentage, etc.

If you are doing full service practice management it's a good idea to include the VOB in your fees. If doing claims only then it's reasonable to charge per service or an hourly fee for these..etc.
