Medicare has payment floor
13 days from receipt of the clean claim for HIPAA compliant Electronic claims and 29 days from receipt for paper claims

No, clean claims of course will be processed first. You can get the payment floor information from your MAC. Won't find a case study because this is a federal payment floor for Medicare.
