Welcome Packet/New Client Packet

Hi! I have 2 prospective clients I am speaking with that are just about ready to sign up. I am wondering, what paperwork do you usually give a new client? I have a contract, BAA, compliance plan, practice/profile sheet but is there anything else I should give to them/have them sign? Also, is there anything I should ask from them when signing them up as a new client? Thanks for any help!

I have pre-made matrix's depending on specialty to get a list of the networks they are contracted with and I also like to have copies of those contracts

I would ask them for a current copy of their A/R, both insurance and patient, and a breakdown of their payer mix. Not sure if you have that already included on your practice/profile sheet you give them, or not. I would also want to see a copy of their new patient form, and patient financial policy form.

Good ideas on your part. Another document that's proved vital to me is a one page client information form, with client's contact information, clinic name, NPI and Tax ID numbers, name of the office contact person, username and passwords, your fee for billing services, business hours, and any other important information. I paper clip this completed form and a copy of the signed agreement together and keep it in a separate drop file. Whatever information your client does not supply, make sure you do. You'll use this document a great deal when contacting insurance companies, creating the monthly invoice for your services, as well contacting your client. A few extra min. spent completing this form correctly when you start will save you a great deal of time and frustration later when you're on the phone and need this info. quickly. Hint: Fill this form out in pencil, as the information supplied may change later on.
