Box 31

Hello - Does a signature have to be in box 31?  If it is a DME supplier billing on the patients behald, is a computer printed name of the company (supplier) with the date acceptable?

When we bill box 31 never has an actual signature, it just has the typed name of our provider.

just to clarify:  For all auditing purposes the provider's signature should be on all encounters.

Even if it prints the dme company and not an induviduals name?  who should sign it then?

I believe she's referring to the patients paperwork/chart.  Not box 31 on the CMS form.

I'm confused....as long as the name itself is printed in box 31, the claim is fine.  An actual cursive signature does not have to be in box 31

I was just clarifying that although the "signature on file" is accepted on the claim, source documents such as the encounter must have the provider's signature.

So just to summarize, original signature must be on source document, but not in box 31.   ;)
