Pain Mgmt - Pump refills using compounded drug

Hi Everyone, 

Can any one tell me the correct way to bill a compounded drug to refill an implanted pump? 

I understand the formula for a single drug, but we have one patient with a compound of three drugs...do I bill each separate, calculating each one by using the formula, and using the individual J-codes? Or do I calculate formula for each one, add the totals together and bill with code: J3490?  I never had to bill with a compounded before, and want to do it right.

It's been a while since I have had this situation, but I would bill each one using the individual J code and calculating the formula.  The J3490 is an unlisted code, so I would avoid it if possible. 

Does anyone else have any experience with this?
