Chiropractic A9900

Michelle,
had a question, we received back a paper from UHC asking for additional information for the use o A9900 or a patient saying that they either need prescription number, ndc number, pharmacy name, drug name as applicable (reasone code 12) or need active or valid cpt-4/hcpcs code and the description, or for anesthesia, the asa/cpt-4 code crosswalk, time in minutes and any modifiers (reason code 05)
Does this mean that they need a modifier or time on this code?

The problem is that A9900 is for a miscellaneous dme supply, so they are looking for either a more specific code that describes the supply, or a description of the supply.  The denial reason is a standard code they use so it doesn't clearly indicate what they are looking for.
