We are a large family/general practice that also provides some OB care to underinsured patients.

Last week, one of our doctor's was doing an antepartum visit and discovered the woman was in advanced labor. He wound up delivering the baby there in the clinic. We also provided the post-partum care once baby and mom were transported to the hospital.

How in the world do I bill this? We don't bill global codes, so all the antepartum visits have been separately billed out.

Do we use a 59409 with the office place of service, and then a 59430 with the hospital place of service? Also, is there a different RVU for office delivery as opposed to hospital delivery? Are modifiers appropriate here?

POS 11 on the Vag delivery and use the hosp POS on the Ante.  OV CPT's are based upon U&C and not RVU so the payment will differ.  If billing just the Delivery no mod is necessary.  Don't see this type of note too much in an OV setting.  Also, if pt. was being treating for something else then ypu s/b able top also bill the E/M.

Thanks! I don't have enough reason to bill a separate E/M, so I've billed out just the delivery with the office place of service.
