Would any one know of a great coding book out there to answer questions like: billing multiple codes for a treatment - how do you know to do that?

That's really a coding question, not a billing one.  But I'm not familiar with a resource for that.  Sorry :(

I agrew with Michele, that is a coding question.  Your primary source should be your CPT book.  Another easy referrence guide is called
