What is A/R (Accounts Receivable) Denial management and can any one explain in detail about the Denials with Remark Code.

This code means simply that some required information is missing.  There should be another code on the denial that explains what information was missing.  Basically the CO16 doesn't tell you much.  You need to be looking at the other remark codes.
