Credentialing For a Hospitalist

Hi, I need assistance credentialing for a Hospitalist.   Because this physician is a Hospitalist, she sees patients in a hospital and does NOT have her own office.   When filling out the Medicare Form 855I, I do not know what to write in Section 4 - "Practice Location Information".  The doctor gave me two addresses...  a UPS mailbox (with a regular address and not P.O. Box) and her home address.   Which one should I enter as the Practice Address Location?   Also, she is in the process of getting privileges to a hospital but it is not Fully Approved yet.  I just called Palmetto GBA and they were no help at all.   ANY help anybody can give me would be very much appreciated!   Thanks in advance!!!

the practice address should be that, where she is delivering services.

Thank you, Regina.   Since she would be rendering her services in the hospital,  I was thinking the hospital address.  But she needs to get approved for privileges first.

her address to send the payment should be different than the hospital address where services are rendered.   she can use her po box if that is how she prefers.
