Using a V code as a primary code for Physical Therapy

It is my understanding that physical therapy coding MAY use a V code but it should not be a primary code.  We put it as the last code.
any thoughts on using or not using the V code

Usually the V code is not the primary reason for the visit.  Usually their is a diagnosis for which the patient is being treated, but also a v code to indicate the cause.  For example, if the patient had hip replacement surgery and is now sent for PT.  There is a v code to indicate the surgery, but the reason for the PT is the diagnosis that originally required the surgery.
