Would I go through CMS to apply for a submitter ID? Any help or advice to offer would be great. Thanks.

You don't go thru CMS, you have to go thru the carrier that administers the Medicare plan for your state.  The submitter ID is obtained by completing the appropriate EDI paperwork which is available on the web site under the EDI section.
