OK, we now have 2 NPI numbers type II NPI link to the practice name and employer tax ID number and type I NPI number linked to the doctor’s name and his personal social security number. When do we use the type II NPI and when do we use the type I NPI, or do we ever need to use the personal, type I NPI number? Thank you for your assistance.

Hi Jill,
If you submit your claims on CMS 1500 forms then you will use the individual NPI (type I) in box 24J to indicate who performed the services. You will use the group NPI (type II) in box 32a and 33a to indicate who the payment should go to.
