standard steps to patient billing

what steps does a patient need to take to file their own insurance claim?   Is the doctor's office supposed to give the patient all the information, (tax id, npi, etc) including the cms 1500 form?

Physicians are obligated to provide a fully itemized billing statement. All of the information they need should be on that statement. You can give them a completed CMS 1500 but many times patient's plans will provide them with their own claim forms they can attach the bill to.  The patient statement should include everything they need, your PM software should accommodate that because like I said, providers are required to produce them.
