CMS 1500 FORM COPY

I am doing billing for the IPT clinic and sometimes i have to use the 1500 forms and mail out the claims. my provider provides the 1500 form.
My provide just sent my an AUTO claim for a patient he's been seeing since JUNE and wants me to bill it ASAP, as patient is still getting Therapy. i had informed my provider that the 1500 claim form is low about a month ago, still havent receive any box of it from my provider.

today, an envelop of "COPIED" black and white 1500 form was dropped off for me to bill the "AUTO" claim with the providers signature...I was in awe and called the provider telling him i can not use a copied 1500...my provider assured me he use to do it all the time, make copies of original and do his claim and got paid.

so please help me, can this be done because with 12 years of billing paper and electronic, i have never used a copy of 1500 or UB04 to file a claim. is this new or im just creating expenses for my provider who is so adamant about providing me with 1500 forms.

Almost all insurance carriers require the Red & White CMS forms.  There may be a few obscure carriers that would take photocopies, but it is generally required that claims are submitted on the CMS 1500 (red & white) forms.

I have never billed from a black and white copy either!  You can run up to Staples and they sell small packs of either 25 or 100(I forget)....I would use originals myself ;)

Is this a provider you work FOR or is he a "client"?  I won't work with providers like that that want to pinch pennies and don't care about right wrong..those are the ones that would ditch you in a heartbeat to outsource to India for $1.00 a claim  ??? I'd simply remind him that he has to follow the rules and his penny pinching ways were going to hurt him.
