Is there a book that has health insurance company names, addresses and phone #s?

or is there a website that I can go to?

There used to be.. check Amazon.. Insurance Company Directory

 but I've always found it best to look at patient's insurance cards..too often you have TPA's involved and you end up contacting the wrong place to verify benefits.

I also have good luck with google when I have one I can't locate or the info I need isn't on the insurance card.

there is a kelly online directory of insurance companies.
