76770 Documentation Requirements

CPT - Radiology 70010-79999 : When a radiology report has a urinary tract pathology and the kidneys and bladder are reviewed this constitutes a complete. If the report has "under filled bladder" would you consider to be completed? The CPT says "the report should contain a description of these elements or the reason that an element could not be visualized and you would code as complete. However according to Dr. Z's Medical Coding Series Diagnostic Radiology Coding Referenc page 203 "When all the elements required for a complete abdominal ultrasound study are not dictated in the physician report, a limited study must be reported. If an area cannot be visualized due to gas or another obstruction, the reason for the inability to view the area should be documented in order to report a complete study. I don't think that an under filled bladder is an obstruction. So can you please advice?
