Negotiating contracts with carriers

Can anyone tell me more info about those carriers that you can negotiate contracts with?

The doctor can negotiate a reimbursement fee with any commercial insurance company except for Medicaid and Medicare.  Their reimbursement fees are fixed per State and Federal Law.
