Need HCFA

Hi all,

I need a "dummy" HCFA filled out to present at a talk I am giving. It's just to show new docs what it looks like filled out...

are there any websites where I can input the info and print one out?

Don't feel like manipulating my PM software to change the acutual doctors' info....

You can fill out this form on-line before printing.  But you cannot save the form with the data typed into it.  You will lose the data.

Hi,

I have one but in a book that we use to train our staff and it's not conducive to scanning without tearing the page out. The other option is to use Adobe Pro X to fill-in if you want to save it (it has the option and will fill-in any document as long as it's not locked from entering). Adobe usually have a trail run of the software before buying.
