Medi-Cal code question: blood draw, urinalysis

hi,
the medi-cal venipuncture code was Z5220 several years ago but i heard that code is no longer valid.  also had to add in cms comments box 19 "blood sent to unafilliated lab".  anyone know the current code?

for urinalysis performed at our office we use cpt 81000 or 81002.  does this cpt need modifier -QW (clia waived test).  also if you're doing both components of test, do you use both modifier -TC and -26?

I've done some Medi-Cal billing and they are a tough one.  I thought they did allow the 36415 now.  As for the QW, I'm not sure.  And I'm not sure about Medi-Cal but when billing for both components to any other carrier you don't use any modifier.
