When billing an insurance company for Speech therapy that was done by a SLPA (not an SLP), whose Provider information and NPI # go on the claim?  Will it be the SLPA or the supervising SLP?  If it is the SLPA, do you bill the same rate as the SLP?

If the SLP is supervising I believe it is the SLP's info (NPI) that goes on the claim, including their rates.
