I wanted to know what the difference was between a UB04 & UB92 and how do I know when to use the correct one. I just started working for an ortho practice and we have both in patient and out patient surgeries.

The UB04 form replaced the UB92 form on May 23rd, 2007.  You should not be using the UB92 form at all anymore.  You would use the UB04 when billing for the facility fees.
