Will there be a problem if the tax ID IRS document does not have the practice address on it?  Currently the provider I work for has a Tax ID for her LLC, but on the IRS doc it has her home address of where she no longer resides.

I think they should update it with the IRS and get a new letter issued ASAP.  You didn't indicate if you are doing anything in particular such as credentialing, but since she no longer lives there it should be updated anyway for security reasons.

The IRS and NPI must match EXACTLY. All i's dotted and all t's crossed. I've seen denials simply for one file having a misspelling.
