Was wondering if anyone had any office notes they would share for examples for allergy testing.  The physician I bill for is considering this option and would like to see examples of some notes.

Sorry, none of our providers do allergy testing.

The "quad A I" (AAAAI) has allergy procedure note templates with examples.

Here are the notes, but you'll also need guidelines and consent form templates and the patient instruction sheets.
